Many school districts and club sports now require a physical evaluation for children to play, though regulations vary by state. If you haven’t already, it’s also a good idea to check with school administrators about specific eligibility requirements. Central Texas Urgent Care have certified medical professionals available for the necessary physical evaluations. At the very least, you’ll be required to sign a waiver, and you may even need to provide immunization records, along with a recent medical evaluation. In addition, be prepared to provide the following medical information about your child:
- Relevant health and medical history
- Dietary restrictions and food allergies
- List of current medications
- Allergies to medicines
- Date of last tetanus inoculation
- Primary medical provider’s name and contact information
